Botox and Fillers: The next generation in treatment styles.

Dr Wade Merchant uses Botox and fillers to enhance the facial aesthetics of his patients. The medical name for Botox is Botulinum Toxin Type A (BTX). They are both injectables but they act in different ways:

BTX: Acts by deactivating muscles that cause wrinkles. The wrinkle then cannot form and the line disappears over time.

Fillers: Hyaluronic acid gels that are injected into the face in order to provide volume to  certain areas where it may be lacking or has been lost as a result of the aging process.

BTX lasts around 3 to 4 months if the recommended units are used – if less than recommended units are used, the results will not last as long as they should. This is important to remember when attempting to reduce the amount of units because of cost – if you use less units now you will need more units sooner, and will probably end up spending more money than if you had just received the recommended units. Medical doctors are specially trained to assess and prescribe the amount of units needed – just in the same way that they know which dose of your anti-depressant to give you.

Filler treatments last different amounts of time depending on the filler used as well as your particular metabolism:

Voluma (Cheeks): 18months.

Volift (Lips and nasolabial folds): over 1 year to 15 months.

Volbella (under eyes and lip border) – 9 months to a year.

At the end of the day, each face requires a different approach and normally a combination of both BTX and Fillers are used.

The number one reason why patients look strange after Botox injections is related to their eyebrows. Eyebrows are an extremely important feature on our faces. They help us to communicate with the world in a non-verbal manner – and according to scientists most of our communication is non-verbal. Botox Somerset West Cape Town | Q&A: Sure, my name is Dr Wade Merchant, I am a qualified medical doctor with a special interest in anti-aging and aesthetic medicine.  I have a practice in Cape Town as well as in Somerset West.  I am originally from Durban but have been living in Cape Town for the past 5 years.

  1. What is Botox and how does it work?

Botox is basically a muscle relaxer.  We don’t inject the wrinkles but rather a small group of tiny muscles that create the wrinkles in the skin.  After the muscles have been relaxed for a while, the skin folds reduce dramatically.

  1. Does it hurt?

Not really.  It’s not like the needles you would see in adverts.  We use a very short and fine needle that most people don’t even feel while having the procedure.  We have a numbing cream that we apply beforehand, but 9 out of 10 patients prefer not to have the numbing cream applied.  Most of our patients say that shaping their eyebrows or having their teeth cleaned is a lot worse than having Botox.

  1. How quickly will it start to work?

Every patient is different, because we all have different anatomy.  Typically 7 days to 14 days after the treatment.

  1. How long does the effect last?

Again, for each patient it’s different.  Typically, 3-4 months with most patients reaching the 4 months mark.

  1. Is it safe?

When done right, yes!  It is critical to have the procedure done with a qualified medical doctor who has experience and the necessary qualifications to inject Botox. We only used FDA or CE approved products at our practice and in South Africa the industry is highly regulated.  Botox have been around since the late 80’s; it’s a trusted product and we see great results with each patient.

  1. Are there any skin care products that we can use to prevent or treat wrinkles?

There is no skin care range or lotion that can prevent or treat wrinkles as effectively as Botox.  A medical approach is the best way to take care of one’s skin.  However, I would always recommend the daily application of sunscreen.

  1. Any other benefits to this treatment?

Surprisingly, we also use Botox to treat migraines, depression, oily skin, gummy smiles, sweaty armpits, bladder problems and in stroke patients to reduce muscular movements.

From a skin care point of view, it is important to know that wrinkles create deep folds in the skin.  Once the folds have been reduced, the skin relaxes as per normal and the effect is a healthier and softer skin.  People suffering from oily or shiny skin use Botox to treat their oily skin and not necessarily their wrinkles. There are many great benefits!

  1. Will I be able to make facial expressions?

Yes, of course.  We only relax a small group of tiny muscles in the targeted area in order to replenish the skin to treat wrinkles.  Most of our clients start by only treating the frown area, on their second visit, they ask us to also treat the wrinkles around their eyes or forehead.

  1. Is there a specific demographic of patient?

Not at all, we see patients from all backgrounds.  Lawyers, estate agents, moms, dads, CEO’s of large companies, small business owners, wine makers, introverts, extroverts, models, fitness fanatics, academics, gay men, straight men, locals, tourists, young and old – there is really no specific demographic.  Most of our clients are confident individuals that like to live a full and healthy life and taking care of themselves is a great priority for them.

Small amounts of Botox are injected into the offending muscles, causing temporary weakness of the muscle, and flattening of the overlying wrinkle. The onset of action of Botox takes 3-7 days, and lasts for about 4-6 months, at which time the Botox treatment can be repeated. In order to get the best out of Botox, Dr Wade urges his Cape Town clients to follow up before the wrinkles return.

More recently, Botox has been used in order to shape the eyebrows, allowing a slight lift of the outer one third of the eyebrow, without the need for surgery. This is also a temporary phenomenon, lasting some 4-6 months. Botox has also found a role in assisting scar revision surgery, by limiting tension on the wound.

Cape Town doctor, Dr Wade uses cosmetic Botox and most of his cases focus on the upper 1/3 of the face. We typically use 20 units  of Botox per pair of muscles (10 units per muscle) and there are 3 possible muscle groups that are routinely injected:

  1. Frown lines (corrigator muscle and occasionally procerus muscle).
  2. Transverse forehead lines (Frontalis muscle).
  3. Crow’s feet lines (orbicularis oculi muscle)

The most common site for Botox in Cape Town is the Frown. Each site typically uses 20 units of Botox, so a maximum of 60 units can be used – all 3 areas. That said, the total amount of Botox used these days to ensure natural results is about 4o units. We also occasionally inject small volumes of Botox into other related sites, eg so-called bunny lines (sides of nose), vertical upper lip lines (smoker’s lines), and sub-brow region (for outer eyebrow lift).

Botox is also useful to minimise sweating in the armpits, and is injected in to the armpit skin at a dose of 50 units of Botox per side (100 units total). Botox is very effective to lessen sweating in this region and the effects may last up to 8 months.
