Redermalization?

Aesthetic medicine can treat facial aging from the level of the bone right up to the skin’s surface. Redermalization targets the skin itself to improve the quality of your skin. There are many treatment options in aesthetic medicine – this is just one of them. The only way to know if it is the best treatment for you is to visit Dr Wade so that he can see your face in person.

Three basic mechanisms of skin aging:

  1. Inhibition of metabolic processes within the skin – cellular respiration, ion transport, inhibition of energy production and proteins (collagen and elastin) synthesis, impaired micro-circulation.
  2. The accumulation of free radicals – uncontrolled synthesis of free radicals, the inefficiency of antioxidant systems, lipid per-oxidation and destruction of the cell genome leading to premature cell death.
  3. Impairment of the optimal water balance in the skin as a result of reduction in the amount of hyaluronic acid in the dermis and reduced ability of the skin to retain water.

Redermalization

What is Redermalization?

Redermalization is an innovative method for efficient aging prevention and skin restoration which affects all the tree main skin aging mechanisms
  1. Regulates biochemical and physiological processes in cells.
  2. Efficiently blocks free radicals.
  3. Prevents skin dehydration
Translated from Latin:
“Re” means “renovation”, “derma” — the basic layer of the skin, that is responsible for its youth. Thus,
Redermalization is restoration of health skin by restoring the dermis.

Dr Wade Merchant Fillers

Fantastic Facial Filler

Facial Filler goes by many names; Dermal Filler, Fillers and Injectable Filler. Let’s take a step back to review what exactly a filler is.

A filler fills. It’s as easy as that – although where it fills and how it fills are the more difficult questions! A filler can either be permanent or non-permanent – that is they either get absorbed over time or they last for life. Non-permanent Hyaluronic Acid (HA) based fillers are the safest type as they can be dissolved.

Lip Fillers

When done correctly, Lip Fillers are about enhancing the lips natural features. Everyone is different and that is why each treatment is completely individualised.

Which brand?

There are many different brands of HA fillers but as I say to all my patients, I only inject the brands into my patients faces that I would inject into my own face. In South Africa we only have a few filler brands to choose from (which is actually a good thing). Our strict laws generally only allow for the best to be injected – and in South Africa, only doctors are allowed to inject Fillers.

Under Eye Fillers

Under Eye Fillers are all about refreshing and rejuvenating tired looking faces. A blunt needle called a cannula is used to deposit the filler exactly where it needs to be – this also limits bruising.

Which filler, where?

They are broadly divided into thick fillers and thin fillers. It is a lot more complicated than that but for our purposes, the thick ones provide structure, support and volume and the thin ones gently fill areas and help to smooth out lines and wrinkles. So…

Thick: cheeks, chin, temples and jawline
Medium thick: nasolabial folds, lips, cheeks
Thin: under eye and lips.
Very thin: lines around eyes, frown and forehead lines as well as acne scars.

So what is best for me?

At the end of the day the only way a doctor can know what to inject and where is to look at a particular face in person. Some people require more volume than others – so the only way to know is visualize in 3D!

I hope this newsletter sheds some light on fillers as a whole. If you have any questions related to fillers or any other treatments that I perform you can contact me by commenting below or emailing me. Have a great week further!

Kind Regards,

Dr Wade Merchant

Eye Fillers

Eye fillers are a treatment that can have an astounding affect on the way a person looks and feels. Many people walk around looking tired and sad –  most of the time this is as a result of the way their face looks. When we age we lose the fat in our cheeks and they begin to sag. The sagging cheeks then pull on the skin just below the eye, and a line forms – known as the tear trough line (named so as it makes you look sad). Before eye fillers came along, the only way to restore volume here was to inject fat that had been taken from somewhere else on your body. Now while a bit of liposuction and some facial rejuvenation are great, they require sedation and surgery – and with that comes downtime, pain, bruising and swelling. Since the arrival of under eye fillers, we have been able to offer clients an in and out service that is relatively painless and has little to no downtime.

Under Eye Fillers

Deep lines under the eyes can be treated with fillers.

The eye fillers procedure takes about an hour – most of that time is spent explaining the treatment and how it works as well as marking out the face to ensure that the correct areas are injected. The markings are made with a white eyeliner – so it easily comes off with a facial wipe – so no need to worry about walking around with lines on your face after your eye fillers treatment! If you are concerned about pain, a topical cream can be applied before the treatment to numb the area. As a general rule, mid face fillers aren’t sore! Below is a video of a doctor injecting eye fillers – it really is a great treatment!

Botox and Fillers: The next generation in treatment styles.

Dr Wade Merchant uses Botox and fillers to enhance the facial aesthetics of his patients. The medical name for Botox is Botulinum Toxin Type A (BTX). They are both injectables but they act in different ways:

BTX: Acts by deactivating muscles that cause wrinkles. The wrinkle then cannot form and the line disappears over time.

Fillers: Hyaluronic acid gels that are injected into the face in order to provide volume to  certain areas where it may be lacking or has been lost as a result of the aging process.

Botox and Fillers

BTX lasts around 3 to 4 months if the recommended units are used – if less than recommended units are used, the results will not last as long as they should. This is important to remember when attempting to reduce the amount of units because of cost – if you use less units now you will need more units sooner, and will probably end up spending more money than if you had just received the recommended units. Medical doctors are specially trained to assess and prescribe the amount of units needed – just in the same way that they know which dose of your anti-depressant to give you.

Filler treatments last different amounts of time depending on the filler used as well as your particular metabolism:

Voluma (Cheeks): 18months.

Volift (Lips and nasolabial folds): over 1 year to 15 months.

Volbella (under eyes and lip border) – 9 months to a year.

At the end of the day, each face requires a different approach and normally a combination of both BTX and Fillers are used.
