Acne Treatments for Scars

Acne treatments are now available – with new technologies we can treat acne scars. Acne scars form as a result of severe acne in the teenage years or as an adult. They can be very disfiguring and can cause some emotional distress to the person that has them. Thankfully there are now acne treatments that work.

The best thing you can do for acne scars is to do microneedling for acne treatments. Microneedling is a treatment whereby the skin is needled and it is stimulated to heal. This healing process makes collagen which then fills in the acne scars. The best microneedling treatment is the Dermapen. The Dermapen is a machine that makes thousands of tiny holes in the skin. Before the treatment you get a numbing cream put on your face – this is so you don’t feel the needles. Usually the most you will feel is a small scratching sensation. Acne Scars

Acne Scars were once a doctors nightmare – but these days, they can be easily treated.  Acne a skin condition whereby the pores in the skin becomes blocked or clogged. The material blocking them is normally dead skin cells or sebum (a type of oil produced by each individual hair).  Interestingly enough, certain medications and creams can actually make acne worse! Most acne sufferers are teenagers – this is as they are going through hormonal changes as a result of puberty. The increase in hormones increases the skins oil production and the pores become blocked. Once the pore is blocked, it becomes irritated and it swells. Swelling can either take the form of a blackhead or a puss filled whitehead. Adults can also suffer from acne – this is known as Adult-type acne.

Mild to moderate acne generally only has blackheads and whiteheads and it generally doesn’t form scars – unless the acne is scratched, pressed or picked at by the person. So as a general rule, don’t ever pop acne pimples – you can form scars. That said, when acne is very bad it is called sever acne – and it is this form that can very easily form scars.

Acne scars form as any scars in our bodies do – as a result of trauma to the skin.  When the skin is broken or you get a cut the body quickly begins to heal itself by forming a scar in that area. When skin is broken and there is infection (like acne) there is a bad type of scarring that occurs – this is the reason for the deep acne scars.

Botulinum Toxin (Botox) is a purified protein derived from Clostridium Botulinum that is used to reduce unwanted dynamic facial wrinkles. It acts by blocking the signal from the nerve to the muscle and in so doing prevents muscular contraction. The effects of Botox last anywhere from 3 to 6 months – it really just depends on the dose, your metabolism and if you have had Botox before. Hyaluronic acid is a naturally occurring substance found in our skin. As we age we lose this substance and our skin loses its once youthful tone and appearance. Aging also results in loss of fat from our facial fat pads. A young-looking face has round contours, high cheekbones and a well-defined jawline. The reason we look the way we do is partly because of the bones in our face but partly due to these fat pads beneath the skin. As they lose fat the skin above them begins to sag and as a result our cheek bones move down our face and we get static lines around our nose and mouth. Skin Cream – Retinol Products

 

Retinol is derived from Vitamin A and has been used on the skin since the 1960’s. Initially it was used in the treatment of acne, but doctors soon found that their acne patients’ wrinkles were disappearing. This led the medical community to study whether or not retinol skin cream could help to treat wrinkles.

Not only did they find that the skin cream successfully treated wrinkles, but they also found that it helped with exfoliation and minor discolorations.

In South Africa we have two main types of retinol skin cream –

Retin-A skin cream made by Janssen Pharmaceuticals and Retacnyl skin cream made by Galderma. There are various concentrations in both brands – the two main types are 0.025% and 0.05%. If you have a sensitive skin type I would suggest going for the 0.025%  skin cream first and then work your way up to the 0.05% skin cream. They are also found in either a gel or cream form. For most patients I would suggest getting the cream form as it has less of a chance of drying out your skin. If a product drys or irritates your skin you will be less likely to use it!

 The Retin-A brand is sold in a 20 gram tube whereas the Retacnyl brand is sold in a 30 gram tube – which is more convenient as you have to make less visits to the pharmacy.

Each skin cream tube lasts around 6 to 8 weeks – only a small amount of the substance is used. Initially I tell clients to use it only 3 times a week at night. Application involves using a small pea sized amount of skin cream on the forehead, each cheek and each side of the neck. Gently rub the skin cream to cover the whole face. Your night serum or moisturizer can then be applied over this. If your skin becomes irritated then step down to twice a week – but if this doesn’t occur you can step up treatment to as many days a week that you like.

Results take a few weeks to kick in – and treatment can be continued for life unless you fall pregnant – Retinol based skin creams cannot be used in pregnancy.

All of these products require a doctors prescription in order to purchase them. You will find that many over the counter anti-wrinkle skin creams have retinol in them. The reason you don’t need a prescription for those is that the concentration is much less – they may sometimes work but it will usually be an expensive and lengthy process.

One of the most common problems that I am faced with in my practice are lines below patients eyes. Many people present to me saying that they look tired all of the time and that they need Botox for the line just below their eye. Patients are often not aware that it is the Tear Trough deformity that is making them look tired all of the time. Some people can be born with this line (and it deepens with aging) or some people can develop it as a part of the aging process – usually from their late twenties onwards. Sun exposure, poor diet and smoking can all speed up the process of developing this deformity.

The only way to correct this deformity is to use a filler to “fill” the space that has been de-volumised. Before we can talk about the method of filling, we must understand why this line develops.

Our faces look the way they do as a result of three main structures – our bones, our muscles and our fat. Interestingly enough, it is the fat that we as aesthetic doctors are most interested in. In our face there are two levels of fat – the deep fat pads and the superficial (shallow) fat pads. Aging results in loss of fat from the deep fat pads. These pads usually support all of the tissue in our face to make us look the way we look. When we lose the fat, the supporting structure is lost and everything moves downwards with gravity. This downward shift pulls on the ligaments just below the eye and they then pull on the skin – the result is a line just below the eye known as the tear trough deformity.

In order to fill this area we must use a filler that is very soft and fluid-like. If we use a hard, gel-like filler we will end up with a poor result as the area will swell and it will be over-corrected. It is important for your doctor to have a clear understanding of the chemistry and physics of fillers before injecting this area. There are two methods of injection – either by a needle or by a cannula (a blunt tipped needle). Cannulas have been shown to cause less bruising – so speak to your doctor when deciding on a treatment method (some practices also charge you for the cannula in addition to the filler).

If a needle is used, 3 to 5 injections below each eye are performed. If a cannula is used, there is only one needle prick and the filler is injected using a blunt needle passed through the first needle-prick site.

The most common question that patients ask Aesthetic Doctors is whether or not they will look like they have had Botox after having had their Botox treatment. What a strange question to ask! Of course you will look like you have had Botox – you will look fantastically refreshed and rejuvenated!

The number one reason why patients look strange after Botox injections is related to their eyebrows. Eyebrows are an extremely important feature on our faces. They help us to communicate with the world in a non-verbal manner – and according to scientists most of our communication is non-verbal. In this type of situation the patient is unable to show the facial expressions of surprise, worry or concern.
