Three basic mechanisms of skin aging:

Dr Wade Merchant Fillers

Fantastic Facial Filler

Facial Filler goes by many names; Dermal Filler, Fillers and Injectable Filler. Let’s take a step back to review what exactly a filler is.

A filler fills. It’s as easy as that – although where it fills and how it fills are the more difficult questions! A filler can either be permanent or non-permanent – that is they either get absorbed over time or they last for life. Non-permanent Hyaluronic Acid (HA) based fillers are the safest type as they can be dissolved.

When done correctly, Lip Fillers are about enhancing the lips natural features. Everyone is different and that is why each treatment is completely individualised.

Which brand?

There are many different brands of HA fillers but as I say to all my patients, I only inject the brands into my patients faces that I would inject into my own face. In South Africa we only have a few filler brands to choose from (which is actually a good thing). Our strict laws generally only allow for the best to be injected – and in South Africa, only doctors are allowed to inject Fillers.

 

 

 

 

 

Under Eye Fillers

Under Eye Fillers are all about refreshing and rejuvenating tired looking faces. A blunt needle called a cannula is used to deposit the filler exactly where it needs to be – this also limits bruising.

Which filler, where?

They are broadly divided into thick fillers and thin fillers. It is a lot more complicated than that but for our purposes, the thick ones provide structure, support and volume and the thin ones gently fill areas and help to smooth out lines and wrinkles. So…

Thick: cheeks, chin, temples and jawline
Medium thick: nasolabial folds, lips, cheeks
Thin: under eye and lips.
Very thin: lines around eyes, frown and forehead lines as well as acne scars.

So what is best for me?

At the end of the day the only way a doctor can know what to inject and where is to look at a particular face in person. Some people require more volume than others – so the only way to know is visualize in 3D!

Forehead Wrinkles

A brief summary of the article written by Dr Riekie Smit in the Summer Issue of the Aesthetic and AntiAging Magazine: found here.

The forehead is one of the most common areas that patients request to be treated. More often than not, there are unrealistic expectations placed on the doctor to take away all of the wrinkles or to make their foreheads completely smooth. The face of the matter is that if you take away all the wrinkles and create a completely smooth forehead, you also take away 100% of the function of the forehead muscle.

So what exactly does the forehead muscle do? Simply put, it lifts up the eyebrows. That said, every face is completely different. Some forehead muscles are strong and some are week, some are asymmetrical and some are symmetrical and finally some serve a purpose (like keeping the eyebrows lifted in people with saggy skin on the upper eyelid) and some have no purpose at all (some males can get away with almost freezing their foreheads – although it is not recommended).

So it is important to keep this all in mind deciding how to treat and how much to treat the forehead wrinkles. so in a nutshell, there are several options:

  1. Botulinum Toxin Type A – This is a good place to start as most of the time people are happy with the amount of wrinkle reduction. If there are still some bothersome wrinkles – a small amount of a thin filler can be used to fill the gaps.
  2. Facial Fillers – the thinnest type of fillers need to be used here – if anything else is used there is the chance that lumps and bumps can develop. Fillers treatments to the forehead and frown must be done with great care – there are complications and patients must be made aware of this. That said, a product like Restylane Vital is great for this area when used correctly.
  3. Peels, Needling, laser and mesotherapy all help to improve the texture of the skin and soften fine lines and wrinkles.
  4. PDO threads increase the collagen content in skin and in so doing help with tightening and fine lines.

Lip Fillers Before and After with Dr Wade Merchant

Dr Wade believes that lips should always look natural – and these lip fillers before and after photos are a testament to that. Not only is every patient completely unique, but each lip filler treatment is completely unique. As we age, volume changes occur. A younger person can not be treated in the same manner as an older person – and vice versa.

As seen above, this younger patient was able to get a volumised appearance with just a single syringe of filler. That is because her foundations were still good – Dr Wade just had to deal with the layout and design of her lips (if we were to continue with the construction metaphor).

As you can see with this more mature patient, a volumised appearance was not possible with only one syringe of filler. Dr Wade focused on her lip border and mouth corners  – so essentially he did “renovations” on the foundations of her lips. She was very happy withe the result as her smokers lines improved and she looked less tired. That said, if she wished to have more volume she would require an additional syringe at a later day.

As you can see – not only are different age groups treated differently, but the results that can be expected are also different. At the end of the day, all that Dr Wade wants for his patients are natural results that make them look and feel better.

What is Silhouette Soft Lift Treatment?

Silhouette Soft Lift treatment is a patented self-anchoring thread technology that permits a minimally invasive approach for elevation and fixation of skin. Silhouette Soft Lift treatment is now available in Cape Town and Somerset West administered under the watchful eye of Dr Wade Merchant. Silhouette Soft Lift treatment is an FDA approved product for elevation and fixation of mid-face, brow and neck.

Silhouette Soft Lift treatment is made from polylactic acid , a material that has been used inside the human body as a suture material for decades. Silhouette Soft Lift is designed with patented dimensional cones that attach to the sub dermal tissue, lifting, suspending and fixating the contour of the brow, mid-face and neck areas.

Procedure Description

The Silhouette Soft Lift treatment is a minimally invasive office procedure performed under local anaesthetic that is designed to elevate, reposition and lift lax skin of the brow, face and neck.  The Silhouette Soft Lift treatment works by having these fine threads inserted painlessly under the skin. These fine threads are similar to those that would be used to support the deep structures of your face if you were to receive a real surgical facelift.

Using local anaesthetic, the threads are passed into the droopy facial element with a special guide needle. The threads are then able to grab on to the droopy soft-tissue and muscle of the desired area. Most patients experience minimal bruising or swelling for 1-7 days. The Silhouette Soft Lift treatment is not painful and most patients can return to normal activities with makeup the next day. The threads stay where they are inserted and, like all internal sutures, are generally well tolerated by the body.

The Silhouette Soft Lift treatment shows immediate effects. Patients can achieve 30-70% of what an invasive facelift may achieve and well over 90% of Silhouette Soft Lift patients report very satisfying lift results. The Silhouette Soft Lift treatment lasts up to 18 month and can be repeated. Many Silhouette Soft Lift patients may combine the skin tightening effects of the Silhouette Soft Less with other procedures. Patients may choose to re-suspend tissue with this technique in several years or go on to more traditional forms of plastic surgery.

What do the threads look like?

Unlike other thread products, Silhouette Soft Lift is clear so they are not visible under fair or thin skin.

How do they work?

Due to the unique design of the cones of Silhouette Soft Less, the surrounding tissues act to hold the threads in place without the need for visible scars. The cones do the work of holding, by being fixated to tissue as they are softly contoured into place. Once in position, your body generates new collagen bundles that surround each thread to maintain the lifting effect.

Does it hurt?

Pain and discomfort are very minimal with this technique and most patients take only Paracetamol for pain relief. In most cases, ice compresses are recommended for the first 24-48 hours.

Is it safe?

The risks and complications are uncommon and usually easily correctable. The actual sutures are made from polylactic acid, which is a material that has been used in brain, heart and abdominal surgery for many years. Since a Silhouette Soft lift is performed under local anaesthesia, there are none of the risks associated with general anaesthesia.

What kind of care is required after the procedure?

Following a Silhouette Soft lift procedure, there are some practical instructions and care that must be followed to avoid traumatizing or dislodging the threads from their supportive locations in the deep fat of the face and neck. Some of these instructions include avoidance of forceful facial rubbing or trauma, sleeping on your back, minimal facial animations, putting pressure on the face, and avoidance of excessive mouth opening. After 3 weeks, there are no more restrictions.

When can I return to work and activities?

Depending upon your job and your tolerance for some minor bruising and swelling, you may be able to return to work in 2-3 days following the procedure. Most patients look good in makeup after one week.

How many treatments will I need?

Usually only one procedure is needed to achieve the desired lifting effect.

How long do the results last?

Physicians’ experiences have shown that duration varies, depending on the age of the patient at the time of the procedure, the degree of sagging, and the number of threads used.

Can this procedure be combined with other cosmetic procedures?

The Silhouette Soft Lift is a stand-alone procedure. However, it is not uncommon to have this procedure combined with Botulinum toxin (Botox) to reduce facial muscle activity, dermal fillers to enhance volume, or peels and laser therapy to improve skin texture.

Are the threads permanent?

Silhouette Soft is absorbable, and it will dissolve over time.

What areas can be treated?

Silhouette Soft Lift is used for:

  • Reposition sagging cheeks
  • Lift drooping soft tissue of the mid and lower face and neck
  • Smooth out premature skin aging

What type of results will I see?

The effects are immediate. During the time of your consultation, Dr. Wade Merchant will demonstrate in the mirror, the relative amount of lift that the Silhouette Soft Lift can achieve. The procedure can also be combined with other techniques including dermal fillers and Botulinum toxin (Botox).

Do I need to have anaesthesia?

In most cases, only local anaesthesia is required. General anaesthesia is not necessary for this technique.

How long does it take to perform?

The Silhouette Soft Lift is most commonly performed under local anaesthesia and the procedure usually takes less than 30 to 60 minutes to complete, depending on the number of threads used. Patients are able to go home soon after the procedure.

Who is a good candidate for a Silhouette Soft Lift treatment?

In general, the best candidates for a Silhouette Soft Lift include both women and men who want a more dramatic and long lasting effect than fillers and Botulinum toxin or other treatments can provide, but do not want a full face lift or brow lift procedure.

How do I know if I am a good candidate?

Ideal candidates are women and men in the thirties through the sixties with moderate drooping of the soft tissues of the brow, face and neck. Patients must have realistic expectations. They should not be too heavy or too thin, and should be medically well and not be taking blood thinning medication.

Forehead Botox

Forehead Botox treatments aim to reduce the amount of wrinkles found on your forehead. The forehead muscle is unique in the sense that it is the only muscle in the upper face that lifts the eyebrows. So, without its action, the eyebrows will be droopy. Therefore, when treating this muscle we need to be very careful so as not to over-treat the area as this would risk lowering the eyebrows, which can actually make you look older.

It is for this reason that Dr Wade has created a treatment system whereby the risk for droopy eyebrows is reduced. As we are all different, and our muscles are all different, there is no clear way of estimating how much forehead Botox will be required. Rather than over-estimating and creating an unnatural result, we place a set dose of Botox into the forehead and then review the results in 2 weeks. If there is inadequate relaxation we inject a micro-dose of forehead Botox in order to get the desired effect.

Botox is FDA approved for frown and eye wrinkles. It has yet to be approved for the forehead – so we use it as an off-label medication. That is, it works to relax the forehead muscles, but it has yet to be approved by the FDA for that area. Clients must understand this fact – it is also the reason why we take a much more conservative approach to avoid any complications. That said, when done correctly, forehead botox works really well to reduce forehead lines and wrinkles.

Dermal Fillers

Dermal Fillers go by many names; Fillers, Facial Fillers, Injectable Fillers – essentially they are all the same thing. And that thing is a very broad category of products that can be very different to one another. Now that I’ve made it look complicated, let’s take a step back to review what exactly a filler is.

A filler fills. It’s as easy as that – although where it fills and how it fills are the more difficult questions! A filler can either be permanent or non-permanent – that is they either get absorbed over time or they last for life.

The current thinking is that non-permanent dermal fillers such as Hyaluronic acid based fillers are the safest to use. As they are not permanent, if you don’t like the result, it will eventually go away. Also, in special circumstances if the dermal filler needs to be removed, it can be dissolved using an injectable enzyme. So for the most part, most doctors today use Hyaluronic Acid based dermal fillers.

And what is Hyaluronic Acid you may ask? Hyaluronic acid is a carbohydrate that is found naturally in our skin. It is a water holding molecule (so it hydrates) and fills the spaces between our collagen and elastin fibers in the dermis of our skin.

Initially dermal fillers were used to fill the lines associated with facial aging. So doctors would typically inject the lower face (nasolabial folds and marionette line). This would reduce the visibility of the lines but it never dealt with the cause of the line – which is facial fat loss.

The current thinking is that instead of injecting the lines in the lower part of the face, we inject the cheeks first – to replace the fat that has been lost. What happens when we do this is that the cheeks are lifted and the lines under the eye, between the nose and mouth and below the mouth are all reduced. If then the person still has lines in these areas, the second treatment would be to directly inject these lines – although most of the time this is unnecessary as the client looks and feels rejuvenated.

The type of Hyaluronic Acid dermal filler we use depends on where we inject it and how deep we inject it. All filler brands have a variety of different fillers within their brand – from the thick gel-like ones to the thin silk-like ones, each has its use. The thicker gels tend to be used for deep injections in the cheeks – they provide the “foundations”. The thinner silk-like ones are injected more superficially – they are like the “curtains”. Foundations of a house provide support and generally last a long time – cheek fillers lift the cheeks (support) and last longer than thin fillers. The curtains of a house get blown around in the wind (thin fillers can easily be moved by the injector) and are usually changed frequently (thin fillers only last around 9 months to a year).

All of the above is just a very small introduction into the world of Dermal Fillers. There are so many more things to know! At the end of the day, one thing stands true: For the most part, it is the injector that you should be concerned with and not the dermal filler choice!

Botox Injection Sites

Botox injection sites are found in the frown, on the forehead and around the eyes. When these sites are injected, the muscles are relaxed and the overlying wrinkles are treated. Different muscles have different functions – so one must be aware that if you relax a muscle too much, it will lose its function and the resulting look will be unnatural. That is why the current trend in Aesthetic Medicine is too inject less Botox and create natural results. You can always visit your Doctor for a top-up, but if you have had too much Botox, you would have to wait 4 months for the effect to wear off.

Having a standard set of Botox injection sites is important for any practitioner – it allows your doctor to treat 99% of the population effectively. That said, some people are born with muscles that are either bigger or in a slightly different place than those found in the text books. What are Lip Fillers?

Lip Fillers are a gel-like substance that is safely injected just underneath the top layer of the lip itself.  Dermal Fillers are made out of hyaluronic acid, which is a natural occurring substance that is found in our skin.

Are Lip Fillers permanent?  

Lip Fillers are temporary, lasting anywhere from 9-months to a year, with most people reaching the 12-month mark.

How much does it cost? 

R3,200 per 1 ml.

How many ml’s would I need?  

90% of our patients choose only  to have 1 ml injected.  Most of the time, this is more than enough.

Can I add another ml or two? 

Yes you can. Because the results are immediate, we will start by only injecting 1 ml. Dr Wade will then show you the results and  you can then decide should you wish to add another ml or two. Alternatively, go home and think about it.  You can always come back in a couple of days should you like to add more.

Why do people get Lip Fillers?

Due to ageing, most people lose volume in their lips, it can be used to restore the volume that was once there.  We have also seen patients that wants to correct asymmetry, patients that wants add volume to match their upper or lower lip, fill-in gaps etc.  Then of course, most of our patients do it to advance their own appearance.  Beautiful lips speak volume, so to speak. 🙂

Eye Fillers

Eye fillers are a treatment that can have an astounding affect on the way a person looks and feels. Many people walk around looking tired and sad –  most of the time this is as a result of the way their face looks. When we age we lose the fat in our cheeks and they begin to sag. The sagging cheeks then pull on the skin just below the eye, and a line forms – known as the tear trough line (named so as it makes you look sad). Before eye fillers came along, the only way to restore volume here was to inject fat that had been taken from somewhere else on your body. Now while a bit of liposuction and some facial rejuvenation are great, they require sedation and surgery – and with that comes downtime, pain, bruising and swelling. The eye fillers procedure takes about an hour – most of that time is spent explaining the treatment and how it works as well as marking out the face to ensure that the correct areas are injected. The markings are made with a white eyeliner – so it easily comes off with a facial wipe – so no need to worry about walking around with lines on your face after your eye fillers treatment! If you are concerned about pain, a topical cream can be applied before the treatment to numb the area. As a general rule, mid face fillers aren’t sore!
