Acne treatments are now available – with new technologies we can treat acne scars. Acne scars form as a result of severe acne in the teenage years or as an adult. They can be very disfiguring and can cause some emotional distress to the person that has them. Thankfully there are now acne treatments that work.

The best thing you can do for acne scars is to do microneedling for acne treatments. Microneedling is a treatment whereby the skin is needled and it is stimulated to heal. This healing process makes collagen which then fills in the acne scars. The best microneedling treatment is the Dermapen. The Dermapen is a machine that makes thousands of tiny holes in the skin. Before the treatment you get a numbing cream put on your face – this is so you don’t feel the needles. Usually the most you will feel is a small scratching sensation. Acne treatments for scars used to be very expensive – these days the treatment has become affordable thanks to the Dermapen.

For acne scars you will need around 4 treatments with the dermapen – one ever month. Acne treatments with the Dermapen typically leave you red directly after the treatment and for 3 or 4 days afterwards. Some people then get a light peel – like a sunburn peel. And by day 7 the skin usually looks like normal. Over the next month, the collagen is produced which then fills in all of those acne scars. Liquid Facelift

The liquid facelift is the non-surgical method whereby the face is lifted using a combination of dermal fillers and Botox injections. Facial aging is as a result of numerous factors namely, genetics, sun-exposure, smoking, diet and exercise. All of these factors cause the face to sag – everything “droops”. More often than not if you are concerned about the lines on your face you will look in the mirror and gently pull the skin in the middle of your face upwards – that is what a liquid facelift aims to do. Gently Lift the face – without the risk or downtime of surgery.

In order to create lift the face must be looked at holistically.

In order to do this your face is thoroughly assessed by your aesthetic doctor. Photos are taken and the problem areas are identified. With this information in mind, your face is cleaned and then marked out with a white eyeliner. Marking the face allows your doctor to identify which areas are deficient in volume. Our faces are never symmetrical (sleeping on a favorite side causes one side to sag more than the other) – marking also helps to clearly show which side has the deeper lines.

The liquid facelift starts with the dermal filler treatment – 3 points on the cheek are injected – this replaces volume that has been lost and also lifts the skin below the cheek. Together with the lifting effect, it also rejuvenated the eyes. As you are probably aware, the first place we look we when we see a face is the eyes. Friends, family and colleagues usually comment on how happy or well rested you look – this is all because of the filler treatment. This lifting lasts anywhere from a year to 18 months. Hyaluronic Acid – the substance found in fillers – has also been shown to stimulate collagen production. So in the months after the treatment, you will continue to look great.

Secondly a Botox Brow Lift is performed – the frown and the tail of the brow are injected. This relaxes the frown (so you appear happier!) and gently lifts the eyebrows. It is a great treatment that will leave you looking refreshed and energized. At the end of the day the liquid facelift is all about facial rejuvenation and less about looking like you’ve have a facelift (which is probably a good thing!).

The treatment itself is relatively pain-less but if you are at all concerned please ask your doctor for a numbing cream to be applied beforehand – they will happily oblige.

Eye Fillers

Eye fillers are a treatment that can have an astounding affect on the way a person looks and feels. Many people walk around looking tired and sad –  most of the time this is as a result of the way their face looks. When we age we lose the fat in our cheeks and they begin to sag. The sagging cheeks then pull on the skin just below the eye, and a line forms – known as the tear trough line (named so as it makes you look sad). Before eye fillers came along, the only way to restore volume here was to inject fat that had been taken from somewhere else on your body. Now while a bit of liposuction and some facial rejuvenation are great, they require sedation and surgery – and with that comes downtime, pain, bruising and swelling. Since the arrival of under eye fillers, we have been able to offer clients an in and out service that is relatively painless and has little to no downtime.

The eye fillers procedure takes about an hour – most of that time is spent explaining the treatment and how it works as well as marking out the face to ensure that the correct areas are injected. The markings are made with a white eyeliner – so it easily comes off with a facial wipe – so no need to worry about walking around with lines on your face after your eye fillers treatment! If you are concerned about pain, a topical cream can be applied before the treatment to numb the area. As a general rule, mid face fillers aren’t sore! Botulinum Toxin (Botox) is a purified protein derived from Clostridium Botulinum that is used to reduce unwanted dynamic facial wrinkles. It acts by blocking the signal from the nerve to the muscle and in so doing prevents muscular contraction. Botox only works on dynamic wrinkles – static wrinkles will need different treatment options. The effects of Botox last anywhere from 3 to 6 months – it really just depends on the dose, your metabolism and if you have had Botox before. The more often you have Botox, you will need less amount of treatments per year.

Hyaluronic acid is a naturally occurring substance found in our skin. As we age we lose this substance and our skin loses its once youthful tone and appearance. Aging also results in loss of fat from our facial fat pads. A young-looking face has round contours, high cheekbones and a well-defined jawline. The reason we look the way we do is partly because of the bones in our face but partly due to these fat pads beneath the skin. One of the most common problems that I am faced with in my practice are lines below patients eyes. Many people present to me saying that they look tired all of the time and that they need Botox for the line just below their eye. Patients are often not aware that it is the Tear Trough deformity that is making them look tired all of the time. Some people can be born with this line (and it deepens with aging) or some people can develop it as a part of the aging process – usually from their late twenties onwards. Sun exposure, poor diet and smoking can all speed up the process of developing this deformity.

The only way to correct this deformity is to use a filler to “fill” the space that has been de-volumised. Before we can talk about the method of filling, we must understand why this line develops.

Our faces look the way they do as a result of three main structures – our bones, our muscles and our fat. Interestingly enough, it is the fat that we as aesthetic doctors are most interested in. In our face there are two levels of fat – the deep fat pads and the superficial (shallow) fat pads. Aging results in loss of fat from the deep fat pads. These pads usually support all of the tissue in our face to make us look the way we look. When we lose the fat, the supporting structure is lost and everything moves downwards with gravity. This downward shift pulls on the ligaments just below the eye and they then pull on the skin – the result is a line just below the eye known as the tear trough deformity.

In order to fill this area we must use a filler that is very soft and fluid-like. If we use a hard, gel-like filler we will end up with a poor result as the area will swell and it will be over-corrected. It is important for your doctor to have a clear understanding of the chemistry and physics of fillers before injecting this area. There are two methods of injection – either by a needle or by a cannula (a blunt tipped needle). Cannulas have been shown to cause less bruising – so speak to your doctor when deciding on a treatment method (some practices also charge you for the cannula in addition to the filler).

If a needle is used, 3 to 5 injections below each eye are performed. If a cannula is used, there is only one needle prick and the filler is injected using a blunt needle passed through the first needle-prick site.

The way we think about facial aging has changed significantly in the last 5 years. Previously, when we saw a line on the face, our first thought was to fill it with something like a dermal filler. For some patients this worked – the line was filled and everyone was happy, but for others, the results were less than perfect.

As we age, the first line that seems to bother us in the lower face is the nasolabial fold – this is the line from the corner of the nose to the corner of the mouth. When fillers first came out, this was the line that everyone had filled. The problem comes when you think about what makes a youthful face look youthful. In a youthful face there is very little volume (forward projection of tissue) in the area between the nose and the mouth. The bulk of facial volume is held in the cheeks.

When you place a filler in a nasolabial fold you run the chance of over-filling the area. This phenomenon has been referred to as the “baboon face” effect. The images seen below are examples of the typical face that results when fillers are injected incorrectly.

The new and correct approach is to look at the face in 3 dimensions. Rather than filling lines, we aim to rejuvenate the face by adding volume where it has been lost. Once facial volume has been restored to what it once was, we see a marked improvement in nasolabial folds – without adding any filler to this area. This is as a result of the lifting effect that the fillers give. The end result is a more natural look.

I like to think of the face as if it’s a 3 dimensional diamond, rather than a 2 dimensional triangle. A diamond sparkles from all angles, whereas a triangle is invisible when turned sideways!

At the end of the day, the lack of a nasolabial fold in a person doesn’t make them look younger or more attractive. As seen in the well-known actress, Blake Lively, in the right circumstances a nasolabial fold can be an attractive feature rather than a flaw. What we notice in her is that she has great cheek volume – this is the reason why she appears youthful. Without the use of fillers in years to come, her cheeks will lose volume and her face will sag downwards. This will deepen her nasolabial folds as well as create a line under her eye called the tear trough. Looking at the face as a whole prevents us from treating incorrectly or over-treating.

Everyone is different and therefore we map out the face using anatomical landmarks. Silhouette Soft Lift

There are a lot of questions out there about the Silhouette Soft Thread Lift. The short answer: it is not a means of hair removal!

Silhouette Soft Threading is the latest advancement in non-surgical face-lifts. Threading has been around for the last ten years. The problem was that the threads were not natural and were never reabsorbed by the body. Silhouette Soft Threads are totally reabsorbable, bio-compatible and biodegradable – which means that you get great natural results without the worry of having a synthetic substance in your face.

The actual threads last 18 months before they are naturally reabsorbed by the body. Before they are reabsorbed, the body is activated to form it’s own natural thread made of collagen fibers. So even though the threads are reabsorbed, the effect lasts much longer after 18 months as a result of this natural collagen stimulation.

The thread is specialised suture – not unlike the same one that you get when a doctor places dissolving stitches in a cut or wound. The specialised part is that there are a series of miniature cones and knots along the thread that give it the ability to hold and pull skin and tissue in a certain direction. Generally, on the face, the direction is upwards and backwards to restore what has been lost to gravity.

Before the treatment you would visit Dr Wade Merchant for a consultation to see where the threads would need to be placed. He will take photos of your face in certain positions so that he can study where the threads would best be placed. The threads would then be placed on your follow-up visit.

Silhouette Soft Lift

On arrival, Dr Merchant would ask you to wipe any excess make-up from your face. You will then sit on a comfortable bed where your face will be cleaned and then marked with a pen so that the correct lines of entry, exit and placement can be adhered to. At this point a small amount of local anaesthetic will be injected with a very small needle into the entry and exit points of the thread. Patients tolerate these injections very well and it is usually done in less than 30 seconds. This is the only “painful” part of the procedure. After these injections all you should feel is movement and pressure.

The threads are gently guided under your skin from entry to exit point with a long needle – none of which is painful as you have already had a local anaesthetic injection.

Once the threads are placed under your skin, they are gently pulled so that the cones on them are activated. These cones then support the tissue in its new position – giving you a natural and gentle lift. The treatment generally takes under 45 minutes.

Generally after the procedure you may feel some tightness and a little discomfort at the sites where the needle entered and exited. The discomfort usually wears off after 2 days – and Dr Merchant generally advises clients to get the procedure done on a Thursday or Friday – you can then go back to normal activities on Monday. That said, there have been some clients that go back to work the following day – everyone is different.

If you only have a mild degree facial sagging then one thread should be enough. If you have advanced sagging, two threads will be needed. Considering the cost of a surgical face-lift as well as the complications and downtime of surgery, this has become a great safe and affordable alternative.
