How Cosmelan works

The Cosmelan depigmentation peel works by inhibiting tyrosinase, an enzyme involved in melanin production. By regulating melanin synthesis, it significantly reduces the appearance of pigmentation while preventing new spots from forming. The treatment begins with a Mesoestetic peel booster, which prepares the skin for optimal absorption, followed by the application of the Cosmelan mask. After a set duration, the mask is removed at home, and the patient continues with homecare using the Cosmelan 2 maintenance cream after 5 – 7 days. This dual-action approach ensures both immediate and long-term results.

When undergoing Cosmelan® treatment at our clinic, you can expect the following:

  • Initial Consultation: During your first visit, our medically trained therapist, under supervision of one of our doctors, will assess your skin and discuss your pigmentation concerns to determine if Cosmelan® is the right treatment for you. 
  • In-Clinic Treatment: The Cosmelan mask is applied in our clinic by a trained professional. This mask will remain on your skin for several hours, during which it works to reduce pigmentation. We will provide detailed instructions on how to remove the mask at home.
  • Post-Treatment Reaction: After removing the Cosmelan mask, your skin may appear red and feel slightly sensitive, similar to a mild sunburn. This is a normal part of the process, and you may experience some peeling as your skin begins to renew itself.
  • Ongoing Maintenance: The home-care phase involves using the Cosmelan 2 cream to maintain your results. Consistent use of this cream is crucial to prevent the return of pigmentation and ensure long-lasting results.
  • Results: Within weeks, you should start to see a noticeable reduction in dark spots and a more even skin tone. The Cosmelan procedure is known for its effectiveness in treating facial pigmentation and melasma.

1 Week Before Your Treatment:

  • Start using SPF daily: Apply sunscreen every day, even if you stay indoors, to protect your skin.
  • Avoid sun exposure: Stay out of direct sunlight to minimize the risk of irritation or complications during treatment.
  • Avoid facial scrubs or exfoliators: Refrain from using any scrubs or exfoliating products to prevent skin sensitivity.
  • Discontinue the following products: Stop using Retinols (Retinoic acid, Retin A, Retinol, Retinoic Acetate), Alpha Hydroxy Acids (Glycolic & Lactic acid), and Salicylic acid to avoid skin irritation.
  • No facial waxing: Do not wax your face during this week; waxing should be done at least two weeks prior to your treatment.
  • For sensitive skin: If you have conditions like eczema, psoriasis, rosacea, or experience stinging sensations or reactions from products, we recommend a consultation to provide guidelines on how to prepare your skin’s barrier for better tolerance of the treatment.
  • Cold sores or herpes: If you are prone to cold sores or herpes, inform us beforehand. You may require two days of oral anti-viral medication before your procedure to prevent an outbreak.

Preparing for the Cosmelan® Procedure

  • Consultation: Begin with a consultation where our Aesthetic Specialist will analyse your skin and prescribe two essential preparation products. These products will be used for a few weeks based on your skin type to prepare your skin for the Cosmelan 1 Mask.

  • Preparation Products:
    • Melan 130+ SPF 50
    • Mesoestetic Brightening Peel Booster
    • Skin Types I-III: 4-week prep
    • Skin Types IV-VI: 6-week prep
    • Included: Melanostop Tran3x Peel 48 hours before mask application.

  • Avoid the following 2 weeks prior to treatment:
    • Exfoliating products
    • Topical retinoids or Vitamin A derivatives
    • Botox and fillers in the treatment area

  • Additional Considerations:
    • Men should avoid shaving two days prior to treatment.
    • No waxing in the treated areas one week before treatment.
    • No peels, laser resurfacing, or medical-grade exfoliation within two weeks of treatment.
    • You should not have active Herpes Simplex blisters or cold sores.
    • You cannot be pregnant or lactating.
    • Avoid photosensitising medication two weeks prior to treatment.

Application of Cosmelan 1 Mask: 

  • The Cosmelan 1 Mask is applied by an Aesthetic Therapist in the clinic.
  • After application, you will return home with the mask on and remove it after the recommended number of hours, as determined by your therapist.

Removal of Cosmelan 1 Mask (Completed at Home by the Patient):

  • Begin by removing the mask from your eyes, neck, and décolleté.
  • Remove the mask carefully by cleansing with plenty of lukewarm water, ensuring you do not exceed the recommended application time.
  • After removal, your skin may exhibit varying degrees of redness, swelling, or flaking, depending on your individual skin reaction.
  • Homecare Recovery: During the 5-7 days post-removal of the Cosmelan 1 Mask, DO NOT APPLY COSMELAN 2 maintenance cream. Flaking may begin 2-3 days after the mask. To ease the flaking, you may use Melan Recovery, Fast Skin Repair, or the Anti-Stress Mask.

Proper aftercare is essential for achieving and maintaining the best results from your Cosmelan® treatment. Here’s how to use Cosmelan 2 and Mesoestetic homecare products to ensure optimal recovery and skin health.

Month 1: 

After the initial 5-7 day recovery period, you will begin your Cosmelan 2 regimen, which includes a 3x daily application for one month:

  • Cosmelan 2 Cream: Apply 3 times daily (morning, midday, and evening).
    Melan Recovery Cream: Follow with Melan Recovery 3 times daily and reapply as needed to soothe and hydrate the skin.
  • Melan 130+ SPF: Apply during the daytime every 3-4 hours to protect your skin from sun exposure.
  • Anti-Stress Mask: Use the Anti-Stress Mask as needed to calm and hydrate your skin further.


Month 2-3: 

During the second and third months, you will adjust the frequency of your applications:

  • Cosmelan 2 Cream: Apply twice daily (morning and evening).
  • Melan Recovery Cream: Follow with Melan Recovery 2 times daily and reapply as needed throughout the day.
  • Melan 130+ SPF: Continue applying every 3-4 hours during the day to ensure consistent sun protection.
    Optional: The Anti-Stress Mask can be used as needed to maintain skin hydration and comfort.


Month 4-6: 

As you move into the maintenance phase (months 4-6), the regimen simplifies:

  • Cosmelan 2 Cream: Apply once daily, at night.
  • Melan Recovery Cream: Follow with Melan Recovery at night and reapply as needed during the day for continued hydration.
  • Melan 130+ SPF: Continue to apply during the day every 3-4 hours to protect your skin.
  • Optional: The Anti-Stress Mask remains an option for added hydration and comfort.


Important to Remember: 

  • Treatment Options After Month 4: Only after the 4th month may you receive normal hydrating facial treatments or the Melanostop Tran3x peels, followed by superficial needling and application of a depigmenting solution.
  • Avoid Certain Treatments: Do not use Derma roller, laser, or radiofrequency treatments for at least 2 months after completing the Cosmelan protocol.
  • Complementary Treatments: During the first three phases, avoid other treatments except for your complimentary LED Light Therapy, which can support skin healing and rejuvenation.
  • Storing Products: Ensure the Cosmelan 2 maintenance cream is sealed tightly and stored below 23 degrees Celsius. If you live in a warm climate, you may store it in the refrigerator to maintain its efficacy.
  • Application Tips: Always apply Cosmelan 2 evenly across your entire face to ensure an even skin tone and texture while also reaping the anti-aging benefits. If there is a significant contrast between normal skin and pigmented areas, consider spot treatment only as necessary.

The Cosmelan treatment involves three distinct phases. The Preparation Phase is priced at R5,500, followed by the application of the Cosmelan Mask and Homecare Kit, which costs R15,750. To ensure optimal long-term results, the Maintenance Phase Kit is available at R8,960.

Cosmelan is one of the world’s leading depigmentation methods that gently targets and treats unwanted pigmentation on the face and body. The two-phase intensive treatment programme safely corrects all stages of the hyperpigmentation process, fading the appearance of existing and stubborn pigmentation as well as maintains and prevents the formation of future pigmentation.

Cosmelan 2 is a homecare maintenance cream that complements the in-office treatment. It helps sustain results by continuing to regulate melanin production and preventing the reappearance of pigmentation. You need to apply it 3 times a day after the initial 5 – 7 day recovery period.

The Cosmelan treatment delivers a brighter, more even complexion while reducing dark spots and pigmentation. It also enhances skin texture and boosts confidence by providing a noticeable transformation.

The Most common types of pigmentation treated with Cosmelan are:

  • Melasma
  • Post Inflammatory Hyperpigmentation (PIH)
  • Sun Spots
  • Acne scarring
  • Freckles

Cosmelan may safely be performed on patients of all ages and ethnicities who are struggling with stubborn pigmentation. This treatment is particularly effective on melasma, a form of hormonally stimulated pigmentation. The in-office procedure typically takes about 30-45 minutes. The mask remains on your skin for several hours, depending on your skin type and condition, and is removed at home.

Most patients see significant improvement after one treatment. However, results vary, and maintenance with Cosmelan 2 is essential for long-lasting effects.

Next day – 4 days

You may start wearing eye make-up immediately but please wait at least 1 week after mask removal and only once your skin has stopped peeling to wear foundation or concealer.

The Melan Protech 130+ SPF does help to cover some of the redness on the skin.

There is no real cure for hyperpigmentation. Epidermal pigmentation may see good to excellent results whilst deeper dermal pigmentation may see fair to moderate results. It is a management process that depends on your commitment to the phases of the Cosmelan Protocol. Failure to persist with the treatment may lead to the reappearance of dark spots.

NoUnfortunately you cannot be on Cosmelan as there are high doses of retinol and salicylic acid in the Cosmelan products.

Topical or oral treatment with isotretinoin, sun-sensitising anti-biotics, hydroquinone, active bacterial, viral, fungal or herpes infection. You have to wait 6 months after topical hydroquinone or oral isotretinoin before starting with Cosmelan.

Preparation Phase- NON-NEGOTIABLE preparation of the skin must be done with Melan 130+ SPF 50 as well as the Mesoestetic Brightening peel Booster. You will use these 2 products at home to prepare your skin for the Cosmelan mask.

Skin Types I-III: 4 week prep

Skin Types IV-VI: 6 Week Prep

Optional: Melanostop Tran3X Peel 48 hours prior to mask application.

Correction Phase- This is done in the clinic by the Aesthetic Therapist. It involves the application of the Cosmelan 1 mask.

 Homecare Recovery- This is the 5-7 days post removal of Cosmelan 1 mask. DO NOT APPLY COSMELAN 2 during these 5-7 days! You may flake 2-3 days after the mask so you may ease the flaking with; Melan Recovery, Fast Skin repair or the anti-stress mask.

 3 Part Regulation Phase- This is your home treatment with Cosmelan 2 Maintenance cream and skin care. 

1 month after clinic treatment (3x a day application)2nd to 3rd month after clinic treatment4th to 12th month after clinic treatment.

Continuous depigmentation

  •  Apply Cosmelan cream -3x Daily
  • Follow with Melan recovery 3x daily and reapply as needed.
  • Melan 130+ daytime, every 3-4 hours

Pigmentation regulation 

  • Apply Cosmelan cream 2x daily (am+pm)
  • Follow with Melan recovery 2x daily and reapply as needed.

  • Apply Cosmelan cream at night
  • Follow with Melan recovery at night and reapply as needed during the day

If you stop after 3 months of the regulation phase then you have not given your skin enough time to calm and reprogram the melanocytes and we find that patients do not see the desired results. Please commit to completing all the phases as per the protocol.

Patients often describe a sensation of heat and stinging. This is all normal.

Cosmelan does not contain hydroquinone or trichloroacetic acid. It is safe for all skin types and safe for our hot South African Climate.

You may use a misting/spray bottle and mist the mask with water to prevent the mask from drying to soon.

8-12 hours depending on your phototype, sensitivity and skin type. Your therapist will guide you with this.

Varying degrees of redness, swelling or flaking. Everyone is different and skins may differ in reaction.

Ensure the product is sealed tightly and store below 23 degrees Celsius. If you live in a very warm climate you may keep it in the fridge.

Vascular pigmentation is driven by blood vessels that are dilated and are activating the tyrosinase enzymes that will stimulate pigmentation. Cosmelan cannot treat vascularity as it cannot extend that deep into the skin. We Recommend IPL after completing the Cosmelan Protocol to shrink down those blood vessels.   • Fading and reduction of hyperpigmentation after the 2nd week of treatment.
  • Evening out of skin tone
  • Brighter, healthier looking skin with reduction in small imperfections such as acne scarring.
  • 2% reduction in melanocyte activity
  • Anti-ageing, wrinkles, oiliness and pore size improvements!
  • Only after the 4th month may you receive normal hydrating facial treatments or the Melanostop Tran3X peels followed by superficial needling and application of a depigmenting solution.
  • No Derma roller, laser, radiofrequency for at least 2 months after completing the Cosmelan protocol.
  • No other treatments during the first 3 phases except for your complimentary LED Light Therapy.

You may continue with the reappearance control phase indefinitely as it has no side effects and has all the active ingredients in it for anti-ageing as well as preventing future reappearance of hyper pigmentation.

You can step down to our Mesoestetic Tran3X homecare products.

Not less than 12 months after the 1st application of the mask. You have to wait a minimum of 1 year as your skin needs time to recover.
Please apply Cosmelan 2 evenly to your whole face to ensure and even skin tone and texture as well as getting all the anti-ageing benefits of the product. Only if the contrast between normal skin and pigment gets to great then do spot treatment application.

Melan 130+ is superior in its ability to protect against UVB rays that actually cause the damage to the epidermis. It also contains Azelaic Acid which has a depigmenting effect and therefore works synergistically with the other products to give you even better results!

It is completely normal for the mask to sting as your barrier has been disturbed by the Cosmelan 1 mask. The Anti-Stress mask will not cause PIH.
